A Medicare Part B participating provider, Equipoise offers occupational services customized to clients’ needs.
Equipoise OT provides Outpatient OT services accepting traditional Medicare B, Blue Cross and Blue Shield of VT, and as an out of network provider with some insurances. Private pay options also accepted. OT services can be initiated with a doctors referral after an office visit, or by requesting a referral from your doctor. We can help explain and assist with the insurance eligibility process.
